As a Stonerise Home Health RN Field Nurse, you will plan and direct services to meet the needs of the patients and familiesYou will develop a working patient care plan and provide direction/supervision to paraprofessional nursing staffSome of your primary responsibilities include:
Completing initial assessments of patients and families to determine home care needsProviding a complete physical assessment and history of current/previous illness(es)Developing a care plan, which establishes goals based on diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions.
Supervising and directing the LPN(s) and/or Home Health Aides in implementing the patient care planRegularly re-evaluating patient nursing needs and revising patient care planAdministering medications and treatments as prescribed by the physicianProviding health care instructions to the patient per assessment and plan of careIdentifying discharge planning needs as part of the care plan developmentActing as Case Manager when assigned by the Clinical Manager and assuming responsibility to coordinate patient care for assigned caseloadBenefits You Will EnjoyFull-time team member benefits include:
